The Wandsworth Project
Supported Accommodation
The Wandsworth Project provides temporary, supported accommodation for single homeless people with a history of poor mental health, having high support needs.
Our staff work in a multi-agency and multi-disciplinary way, alongside mental health professionals and other agencies involved in an individuals’ care.
Our staff promote an asset/strength-based approach towards client engagement and the autonomy of their own care and emotional well-being.
At Wandsworth, each resident has an allocated key worker to help co-ordinate the care and support they need.
Through structured co-produced support planning and liaison with Community Mental Health Teams, other health care professionals and a client’s own assets (as per the DoH’s Triangle of Care), staff encourage residents to get involved in all areas of their own care.
Staff support their clients to develop the life-skills needed to engage in ETE (Education, Training and Employment) initiatives, as well as developing the self-confidence to sustain themselves within the community.
Need Details
Referrals into the project come via the Local Authority and residents stay at the Wandsworth Project for a period of up to two years.
Those individuals referred into our service can expect to receive the following care and support:
A designated key worker
Regular support planning and reviews
Support to attend all primary and secondary care appointments
Assistance to pursue ETE initiatives (or develop their own business ideas)
Development and assessment of daily living skills
Help maintaining a tenancy
Finance and budgeting
Building confidence, resilience & self-esteem
Three months post-resettlement support
Community engagement
On departure from our service, former residents can also receive ongoing support for up to three months after they leave, to help them settle into their new permanent home.
We operate a 24-hour service with dedicated staff to respond to calls and queries within the operational hours and emergencies or concerns during the out of hours service.
